1. Who Provides Services under CDPAP?

Individuals living with intellectual, developmental, and physical disabilities may need assistance with daily medical care and living tasks. Traditionally, home care agencies provide these services, but with CDPAP, the participant or their guardian has the option to choose their caregiver. This allows individuals to hire family members, friends, or any caregiver of their choice. It also means that the individual or guardian is responsible for all duties required when hiring an employee, such as training and supervision.

2. What Services Are Covered?

Under the CDPAP, any services completed by a home care aide can be provided. This may include:

  • Medication management
  • Feeding assistance
  • Wound care
  • Bathing and dressing
  • Personal care
  • Meal preparation
  • Transportation
  • Light housekeeping
  • Feeding assistance
  • And more

3. Who Is Eligible for CDPAP?

There are seven eligibility requirements individuals must meet to qualify for the CDPAP:

  1. Require medical assistance
  2. Eligible for long-term care by a home health agency, AIDS home care program, personal care services, or private duty nursing services
  3. Have a diagnosed and stable medical condition
  4. Have a designated representative or be a self-directing participant
  5. Require assistance with medical care tasks, personal care tasks, or skilled nursing tasks 
  6. Complete responsibilities as a self-directed participant or have a guardian or representative fulfill those responsibilities on their behalf
  7. Commit to the required assessment and all reassessments or allow the guardian or designated representative to participate
