The intersection of home care and home health care means that the mobile technologies empowering home care agencies are also being adopted by medical care providers. To meet the growing demand for quality home-based care, the Home Centered Care Institute recently launched the HCCI Centers of Excellence for Home-Based Primary Care.

What the HCCI Initiative is Focusing On

With the HCCI initiative, partnering institutions will be training practice managers and clinicians with a home-based primary care curriculum. This curriculum has been developed by a group of nationally recognized leaders in health. The curriculum will focus on:

• Foundational principles
• Economics
• Operations
• Clinical care

HCCI’s goal with this initiative is to increase the number of home-based primary care workers by 5,000 by 2022.

Who is Participating with this Initiative?

Participants in HCCI’s initiative include some of the major health care providers in the United States, such as Cleveland Clinic, the University of Arizona Center on Aging and the Icahn School of Medicine at Mount Sinai. Mount Sinai has been working with Contessa Health to expand its care at home program, Mobile Acute Care Team (MACT). Currently, the program is limited to Medicare patients with certain acute medical conditions that normally would require inpatient care.

Keeping Aging Adults and Chronically Ill Patients at Home

New mobile home-based care technologies are helping keep older adults and chronically ill patients in their homes with cost-effective home care. These technologies include:

• Remote sensors and monitors
• Blood pressure cuffs
• Telemedicine
